2020 Hoboken Wellness Crawl Application
We are so excited you're applying for the Hoboken Wellness Crawl 2020. This year's Wellness Crawl will take place on Sunday, April 26th from 11AM-4PM.

We will be accepting application forms through Friday, February 7th, 2020

This application form, if accepted, will be used as Hoboken Girl's statement of work.

Applicants will be notified by Wednesday, February 19th, 2020 of their acceptance

Contracts + Payment will be due by February 26th, 2020.

All Wellness Table vendors must provide their own table and chair (maximum size 4 x 6). Please confirm you are able to do this. *
The Wellness Hubs require load-in of your materials/goods up flights of stairs (no elevators provided). Please confirm you understand this and are able to load-in your table + day-0f materials. *
Do you plan to host classes at your studio/gym during the crawl? (Classes MUST begin after 11:15AM) *
Vendors must be open for the ENTIRE crawl (11AM-4PM) on Sunday, April 26th, even if this is outside of your normal business hours. Please confirm you will be open throughout the entire crawl. *
Hoboken Girl may use an image from my business' Instagram account or business' website for promotional purposes for the Wellness Crawl. *
There is a $225 fee (+3.6% service charge) to participate in the event. Please confirm you're aware of the fee. *
All invoices must be paid online via credit card by February 26th, 2020 at 12:00PM. No checks will be accepted. Please confirm you will pay with credit card. *
My business must fill out and send the Hoboken Girl Standard Contract by Wednesday, February 26th at 12:00PM in order to be included in the Hoboken Wellness Crawl

Applications will be accepted through February 7th, 2020. Applicants will be notified by February 19, 2020 of their acceptance.
