Use this form to register a one-time change from your normal route OR to register a seat using a Ten Pack Single Ride Bus Pass (which can be purchased at the Business Office first) for the 2023-2024 school year. Please call the business office for information on the the 10 pack single ride bus pass at 831-768-6112.

Please note that this form represents parental permission for the pick-up and drop-off locations requested.
