In accordance with Ontario’s COVID-19 self-assessment as of January 6,2021, we are requiring every person that will enter the Hawthorn School building to first complete this form. Failure to submit this form will prohibit the staff/student/guest/visitor from entering the building.  Please note that we based the questions on the Ontario Screening Tool however, our IPAC Policy are more strict to ensure safety.

The updates to the school screening tool align with the Ministry of Health’s new guidance for individuals who may be experiencing post-vaccine symptoms. In particular, individuals who are experiencing certain minor symptoms that are compatible with COVID-19, but have received a COVID-19 vaccine in the past 48 hours, may still be able to attend school if these symptoms began after vaccination.

Please note that the form is to be filled in immediately prior to coming to school in the morning and not the night before. We thank you for your cooperation and dedication to keeping our school safe.
