Absence Request Form
Each student, in consultation with her/his family, is requested to submit one form per each absence date from a Black Raider Band event. Please consult with the handbook (found on band website) for attendance and schedule expectations.

This form requests being absent from an entire or missing any part of a rehearsal or other event. For each event, there is a section to note if the student will be Absent, Arrive Late, or Leave Early. If Arrive Late or Leave Early, the student will note the estimated number of minutes. The student will also need to state the reason for the request.

A band director or sectional staff will speak with students if more information is needed. Requests for fall season MUST be submitted by August 10th (last day of camp). Emergency situations will be handled on a case-by-case basis.
