Use this form to report a sub. This form should only be filled out by the team captain.
Captains must sign in subs prior to 6:00 p.m. in order for the sub to be eligible to play.
Only players who's names appear on this form are eligible to sub. If you have someone you would like to sub and their name does not appear in the drop down list for your level, ensure your sub puts their name on the League Sub List. This should be done in advance as the League Director will need to verify the player has a waiver on file AND assign a level. Once this occurs the sub's name will appear in this form.
