P&L Make-up Class Form
Make-Up policy: 

• A minimum of one (1) hour advance notice is required to qualify for a make-up lesson.
• Notice must be provided by email, voicemail, or phone call.
• All make-up lessons are subject to availability.
• A completed Make-Up Request Form is required to schedule a make-up lesson.
• Make-up lessons cannot be rescheduled once booked.
• Make-up lessons cannot be carried over to a different semester.


Make-Up Allowance by Pass Type:

• 7-Week / 8-Week Pass: Up to 2 make-up lessons permitted with one hour notice.
• 4-Week / 5-Week / 6-Week Pass: 1 make-up lesson permitted with one hour notice.
• 3-Week Pass: No make-up lessons permitted.

Make-Up Pass can be applied to:

• A different Play & Learn class in the same semester
