Fall Music Online Registration Form 2024-25 Classes begin Monday September 16, 2024
Thanks for your interest in music study this Fall. Please use this form to formally register for the following programs. In addition to the registration form, an invoice  will be sent with the total cost.First year students may select a 3 payment plan option with a credit card oin file. A $300 deposit is due  and registration fee ($60) in order to confirm your private lesson time and day. Any questions? Call us at 631-271-4626


Payment details:
- Tuition is non-refundable. Once registered this is an agreement that the full amount of the tuition for the year must be paid whether the lessons are completed by a student or not. 
- For monthly or 3  payments, a credit card must be current and on file.
- Payments made with credit card with carry a 3% annual fee which will be added to total
- There is no cost to use Venmo/Paypal/Check
- 10% discount for more than one 32 week program registration
- 10% discount for 2 or more siblings registered for a 32 week program
- 5% Paid in full at registration
- No discount can exceed 15%

 $300 deposit + $60 per registration
