Remote Damage Assessment Request Form
No matter where you are in the country, we can estimate the damage to your exercise equipment from afar. We are educated and experienced in equipment being affected by: Flood, power surge, moving, delivery, etc. Our damage assessment consists of an estimate for repair cost, replacement cost (if applicable), and likelihood of repair.
This assessment may be submitted to insurance companies as part of the claims process. In some instances, your insurance company may also reimburse you for the cost of our damage assessment.

In order to proceed, please provide us with the make/model of your machine, along with photos, and purchase price of your equipment. If more convenient, you may email us this information at info@treadmillfixation.com. PAYMENT MUST BE SUBMITTED TO US IN ORDER TO RECEIVE A DAMAGE ASSESSMENT.
