This form is to be completed by the Team's Project Leader. The Team's Project Leader agrees to be the recipient of the Prize Allocation, in the case of the Team that they represent wins a prize. A Team's Project Leader may submit one or more forms and may participate in or lead one or more teams. Team members may not be judges.

An updated form submission may be made by updating this form, or submitting a subsequent form to replace this, until the Submission Deadline. Please indicate below, if this is an updated form.
