CSDI Space Reservation Form
Thanks for proposing an event or meeting to be held in the Center for Student Diversity and Inclusion!

Please use this form to formally request either the Main Space or the Conference Room for your event or meeting.

Follow all instructions carefully and fill out all the required fields. If The Center is able to accommodate your request, a member of the staff will use the information you provide to contact you within 72 hours of your request.

This form should be used specifically for events or meetings planned at least two (2) weeks in advance of the proposed event or meeting date. The Center staff will review requests for more immediate events and meetings on a case by case basis.

To minimize workplace disruption and allow for full use of The Center's Main Space and Conference Room, the full duration of any event or meeting may not exceed 2 hours, including setup and clean up time.

Any events and meetings sponsored directly by the Center for Student Diversity and Inclusion will take precedence over all other proposed events. That said, we at The Center will do our best to accommodate all proposed events and meetings that align with our ongoing programs and initiatives that support diversity, equity, and inclusion for the student body at Carnegie Mellon University.

Only students completing this form who have read and will comply with The Center's space reservation policy may have their reservation request considered for approval. Compliance extends to the student organization and all expected attendees for the event or meeting for which the space is being requested.
