Overview of Officiating Skill Document Request
Is this the right form? This form is used create an Google Doc in which you can complete an overview of an official's skill (OOS), for use in certification review. This form should be completed by someone familiar with the official for at least 6 months. It could take a couple of hours.

Please note that any information submitted to Certification via any channel (Evaluation form, OOS, email, or Feedback on Character) could be shared with the grievance committees of various governing bodies (WFTDA, MRDA, JRDA, and NGBs). The content will also be used in review summaries in an anonymous and paraphrased form.

Steps to complete the Overview:
1. Fill out this form -- we will create and email you the doc to complete.
2. Complete the doc.
3. At the top of the doc, there will be a line that says, "INCOMPLETE: Remove this line after you have completed your Overview and it will be automatically submitted."
4. When you remove that line, the document will automatically submit itself and you will lose access to it. You will receive an email confirming its submission.
5. After you have lost access, please email certification@wftda.com and explain the situation if you need to update the doc again.
