As a member of the Baltimore Celtic SC community, I must answer the following screening questions prior to attending any sanctioned Baltimore Celtic related training and/or competition event.

Instructions:
All questions must be answered 2-3 hours prior to any Baltimore Celtic in-person event.  In the event you experience an issue with reporting, please contact your coaching staff for instructions.

If you answer NO to all questions relating to symptoms, please proceed to your Baltimore Celtic SC event.

If you answer YES to any of the questions, please contact your Baltimore Celtic coaching staff immediately, DO NOT attend any Baltimore Celtic SC event until further instructions are provided.
