Classroom Treat Order Request
Happy Birthday!  Use this form to order treats to be sent to your child's classroom to celebrate their birthday.  Once you have filled out this form, a member of our team will verify the number of students and the date with the teacher.  If there are multiple birthdays in your child's classroom, we will leave it up to the teacher if they'd like more than one snack on the same day or celebrate one of the students a day before or after.  If you have questions, call 746-4328 or put them on this form.  

The cost for treats is .75 per person and we send enough treats for the students and adults in your child's classroom at snack time.
