I understand that I must be 18 years or older to fill out this form. I also understand whether I pay in full (down payment included) or not, a non-refundable down payment is part of this registration. Because the down payment covers flights/tools/materials purchased prior to camp, I understand that I will forfeit that down payment if I choose to not attend The Broadway Academy anytime after completing this registration. If I choose to pay only the downpayment, I understand that The Broadway Academy (contact@broadwayacademy.org) will send me a follow up email up to two weeks prior to camp with a request for the remaining balance owed. If I fail to pay the remaining balance a week prior to camp, I will forfeit my $100 non-refundable down-payment and the student will not be able to attend this year's camp.
