INFORMED CONSENT

You are invited to participate in a market research survey conducted by our small business, Pacifica Food Hall. The purpose of this survey is to gather insight into business preferences, which will help us improve our business to business experience. Your participation is entirely voluntary. 

The survey consists of a questionnaire containing 10 questions about the food hall and your preferences. Participation is not time-limited.

This survey will answer questions, aiding our market research. The data collected will be used to develop a marketing and business plan, providing a solid foundation for our new business.

There are no specific risks of harm associated with participating in this survey. However, respondents are asked not to answer any questions that they feel could put them at risk of harm.

Participants in the survey are free to withdraw at any time, for any reason, without penalty. All responses to the questions contained here will be treated at the highest level of confidentiality. Identifying characteristics will not be associated with specific responses, and all reported data will be presented in its aggregated form and grouped with other responses after statistical analysis.
