The Safety and Incident Report form is used by the PDD Board to address specific incidents and ongoing safety considerations to monitor and improve dog and human safety at the park. For any incident related to fights, we strongly recommend exchanging contact information. NOTE: Completing this form does not substitute for filing a police report.

PDD is a volunteer-run organization. We rely park users to follow posted rules and monitor their dog(s)'s behavior at all times, and we encourage members to direct other park users to the posted rules as needed. The dog park is used at the discretion of dog owners and owners are legally responsible for the behavior of their dog(s) at all times.
