I hereby authorize my child to participate in the Agape Faith Family Festival Unity Choir including workshops, camps, rehearsals and performances. I further authorize making use of any films, photographs or other recordings of these activities for any purpose that FFF may make or authorize to be made without compensation to my child or me. I also authorize use of my child’s name or photograph, video in publicity information pertaining to the performances. This includes radio, television, print, internet (Facebook, YouTube, Instagram, X, website). I affirm that I have the authority to sign this consent.

Faith Family Festival Children & Youth Choir is a diverse and welcoming ensemble for young people of all backgrounds and beliefs. I understand that the FFF Choir is a gospel choir that consists of all Christian songs. I also understand that rehearsals will consist of Biblical Devotions, praise and worship, church history of gospel/Christian music and will open and close in Christian prayer.

Parent Agreements:
Attendance: Children participating in choir must be signed in by adults and attend rehearsals regularly.
Discipline: Participation is a privilege. Disruptive or
disrespectful behavior towards other students or volunteer staff is cause for removal from choir.
Student Pick-Up: Children participating in the choir must be signed out and picked up on time.
Parental Support:  You are an important partner in our choir's success. Therefore your help is needed to make the rehearsal experience the very best it can be.
