ASSAP Streetlight Interference Data Exchange Questionnaire
This form is intended to report unusual experiences involving streetlights turning on or off in the vicinity of the witness. Such strange phenomena known as SLI was investigated at great length by Dr Hilary Evans and ASSAP: the questions on this form are derived from the modifications suggested by Gary M. Rowe.

Please note that faulty streetlights can be extremely dangerous with a risk of electrocution even in close proximity, so do not directly approach a streetlight that is behaving oddly to investigate, but report to the local authorities.

Data submitted to this form will be held confidentially except in the section where noted: your description there may be published if you choose to grant permission for the purpose of analysis. Aggregate data and analysis will be shared amongst the research community, but not personal identifying details.

You may choose to withdraw your entry from the database at any time by emailing chair@assap.ac.uk and all data held by us regarding your case will be deleted.
