Event Description:

The Voices for Change: A Youth Summit for Peace is a youth-led event designed for middle school youth. This summit will feature engaging activities including panel discussions, artistic performances, expert sessions, and interactive workshops. Our goal is to address the rising culture of violence among youth by fostering open dialogue, sharing personal stories, and exploring constructive solutions in partnership with community leaders and local organizations.

  This form requires parental consent to ensure liability, media release, and safety. By completing this form, you agree to the requirements set forth by Hampden CHIP’s Violence and Injury Prevention Community Team.  

Pick-Up & Drop-Off Instructions

Parents or guardians are required to drop off and pick up their child. Only the individual(s) listed in the emergency contact section of this registration form will be allowed to pick up the child.

  • Drop-off Time: 3:00 PM
  • Pick-up Time: 7:45 PM

For the safety of all participants, identification may be required at pick-up. Please ensure timely arrival for both drop-off and pick-up.

Voces por el Cambio: Cumbre Juvenil por la Paz es un evento dirigido por jóvenes y diseñado para estudiantes de escuela intermedia. Esta cumbre contará con actividades interactivas, incluyendo paneles de discusión, presentaciones artísticas, sesiones con expertos y talleres participativos. Nuestro objetivo es abordar la creciente cultura de violencia entre los jóvenes, fomentando el diálogo abierto, compartiendo historias personales y explorando soluciones constructivas en colaboración con líderes comunitarios y organizaciones locales.

Youth Transportation:
I understand that I am solely responsible for providing transportation for my child to and from the event. I will ensure that my child arrives at the event venue on time and is picked up promptly at its conclusion.
