We are updating and consolidating internship placement information. All interested students must complete this form, even if you already submitted information earlier or already have a possible employer.

This form will help us confirm your school, counselor, parent/guardian contact information, internship interest area, employer/placement status, transportation, CTE course alignment, preferred semester, and mentor teacher status.

Completing this form does not guarantee placement. Internship participation requires student eligibility, CTE course alignment, schedule approval, transportation, employer/worksite approval, mentor confirmation, completed paperwork, and final approval from the Work-Based Learning office.
