Parent Questionnaire

Welcome to SVMS!  The past year and a half has been challenging for all of us.  We hope that this year will bring some normalcy back into our school routine, but regardless of what happens this year, you can be sure that the Fifth Grade Team of Teachers will work hard to provide the most positive experience possible for your child.  The best thing you can do as a parent is to communicate any concerns that arise with your child's teacher.  The sooner problems are addressed, the better the outcome will be for everyone.  Any concerns that you share on this form will be shared with any of the teachers who need to be aware of the concern.
