NUAMES Course Surveys and School Climate Survey Parent Notification
We want to inform Parent/Guardians that throughout the school year, your NUAMES student will participate in various surveys conducted within their classes, coursework and NUAMES administers the USBE School Climate Survey to students and stakeholders. These surveys are designed to gather valuable feedback on their learning experiences, NUAMES instruction, school environment and overall stakeholder satisfaction. This feedback is crucial for us to enhance our programs and better support your child’s education. Participation is voluntary, results are anonymous and are kept confidential. The law requires parents to opt-in to participating in surveys, such as the School Climate Survey, during the school year. We appreciate your feedback and support in helping us create a positive and effective learning environment. 

General Information

The School Climate Survey is an anonymous survey used to assess stakeholder perception of a school environment (53G-8-802 and R277-623) and is administered to students and adults. NUAMES will be using the 6-12 grade version of the survey that students can participate in. Adults can participate in the survey that aligns with their role. Adults survey versions are: administrator, faculty and staff, school resource officer, and parent.

How the Data Will be Used
  • NUAMES may use the data to: 

    • Revise practices, policies, and training to eliminate harassment and discrimination

    • Adopt a plan for harassment-free and discrimination-free learning

    • Host outreach events or assemblies to inform students and parents of plans adopted from the review

  • The Utah State Board of Education may review data to:

    • Inform or guide state policy

    • Offer professional learning to districts and charter schools

    • Provide technical assistance to districts and charter schools

  • The public may view summarized survey data (e.g., aggregated results) for their awareness and information.

Data Collection Information

Data collected for the School Climate Survey is housed by Qualtrics, while access to the data is managed by the district or charter and the Utah State Board of Education. Information about Qualtrics and Merkle can be found at:


Who will have access to my student’s responses? 
Each student’s response will be anonymous with no name associated with their response. However, several entities will have access to those anonymous 

individual-level responses as listed below: 

  • NUAMES primary survey administrators and other Qualtrics users involved in administering the survey

  • Utah State Board of Education primary survey administrators of the Qualtrics survey

  • As needed, NUAMES and Utah State Board of Education survey administrators may grant temporary access to survey data to Qualtrics and Merkle (e.g. for technical assistance)

  • Auditing agencies, as applicable. Agencies with the right to audit the data include the Office of the Legislative Auditor General or the Office of the State Auditor


Who will have access to summarized (e.g., aggregated) survey results?
  • Summarized state-level results are accessible to the general public via the Climate Survey Data Dashboard.

  • Summarized district- or charter-level results are accessible to: 

    • District or charter primary survey administrators and other district or charter staff involved in the administering of the survey

    • Utah State Board of Education primary survey administrators of the Qualtrics survey and other Utah State Board of Education staff involved in administering the survey

    • The school community, if published by the district or charter

  • Where applicable, summarized school-level results are accessible to district or charter survey staff

    • NOTE TO LEAs:  LEAs must provide information in this section about who will have access to school-level data: 

      • If school-level aggregate results are made available through a school-identifier included in the survey, AND/OR

      • When the LEA has a small number of schools
