I, on my own behalf and/or on behalf of my child/ward named herein, understand that participation in the Zikr Dance Ensemble Masterclass at Colorado Mesa University ("CMU") involves inherent risks of injury, including but not limited to falls, sprains, strains, and other potential harm. I voluntarily choose to participate and accept these risks, and I hereby release Zikr Dance Ensemble and CMU, and their instructors, staff, and volunteers from any and all liability for any injury or damage suffered during such participation, whether or not due to negligence. I affirm that I/my child/ward are physically fit and medically cleared for participation, and this waiver is signed freely and knowingly. *
