Congregational Membership Application

!!!  --  PLEASE READ BEFORE APPLYING  -- !!!

This application is for the start of your Probationary Period as a congregant with TST Kentucky.  This application DOES NOT guarantee congregational membership.  By filing this application, you are agreeing to fulfill the following requirements:

1.  You must be 18 years of age, a legal adult that resides in the state of Kentucky.  
2.  You agree to a 6-month probationary period that is to start on the Date of Application.  
3.  Within this period, you must gain sponsorship from two of the Council Members. (Listed Below)
4.  You agree to attend 3 public social events of TST KY.  Attendance at online or virtual events will count towards membership at a half rate.  Example: I have attended 2 Temple Discussions posted on the Facebook group.  I have 1 out of my 3 attendance requirements.  
5.  You agree to a final interview or alternate essay submitted to Council.  

Applicants may re-apply or request appropriate accommodations from Council.
