PLEASE NOTE:  In order to provide the best service and a one-on-one experience, adoptions are done BY APPOINTMENT until further notice.  Our adoption center doors are open to the public for viewing animals during our regular hours but you will need an appointment to interact with the animals and finalize adoption.  We have many animals in need of homes and this adoption questionnaire is the first step to schedule your appointment.  Available animals can be found on our website at http://saveapetil.org/adopt/.  Some new animals may not be listed on the website or some listed there may already be adopted.  For the most updated information, feel free to call our Adoption Staff at 847-740-7788 ext 100 or email adoptions@saveapetil.org.  

This adoption questionnaire typically takes about 10-25 minutes to complete and then will be submitted electronically for review by a Save-A-Pet Adoption Counselor for animals available for adoption at Save-A-Pet in Grayslake, Illinois.  If you close this window before submitting the Questionnaire, your responses/progress will NOT BE SAVED.

Completing this questionnaire does not guarantee or reserve any animal under Save-A-Pet's care.

If a dog adoption match is not found at this time, this Dog Adoption Questionnaire is good for up to 1 year.  Save-A-Pet reserves the right to restrict or deny any adoption.

Potential adopters should be prepared to present a photo ID with current address at the time of adoption.  If different, proof of current address will be necessary.

You must be at least 21 years of age to adopt a Save-A-Pet animal.

All family or household members who will live with the animal(s) may need to be present at the Adoption Center to interact with the animal(s).  If extraordinary circumstances prevent this from being possible, please let our adoption counselors know.  All children under the age of 12 in the household are required to meet the animal(s) in all situations.

If you rent the home you live in, you will need a copy of a lease or verbal confirmation from your landlord regarding your pet policy.  Condo/Townhouse owners will need to present a copy of their bylaws.
