2018 Tour de Temple Registration
Registration for riders, leaders, and other participants. The Church's Permission and Medical Release Form must also be completed and submitted to your ward young men president prior to beginning the ride. See  http://bit.ly/ldspermissionslip

Adult participants need to submit a $40 payment to their ward using a donation slip/envelope. Cost for participation for youth members in the Indianapolis stake are covered by the stake.  Cost for other participants outside of the Indianapolis stake is $40

How will you participate in the Tour de Temple? I will be a: *
I hereby assign and grant to the Church of Jesus Christ of Latter Day Saints (LDS Church) and Intellectual Reserve, Inc. (IRI) the right and permission to use and publish the photographs/film/video tapes/electronic representations and/or sound recordings made during youth conference of myself and all individuals associated with this application. I hereby release the LDS Church and IRI from any and all liability from such use and publication. If a minor, by the signature of my parent or legal guardian below, my parent or legal guardian further assigns and grants to the LDS Church and IRI the right and permission to use and publish my image, and hereby agrees to indemnify the LDS Church and IRI against any actions taken by me. I and my parent/guardian (if applicable) hereby authorize the reproduction, sale, copyright, exhibit, broadcast, electronic storage and/or distribution of said image without limitation at the discretion of the LDS Church and IRI, and we specifically waive any right to any compensation we may have for any of the foregoing. *
