Use this form to register for swim lessons.  Group lessons are $150 per session.  Each session consists of 6 lessons over a 2 week period.  Lessons will take place on Monday, Tuesday, and Thursday of each week. Please complete a form for each child you are registering.  Payment can be made at the pool prior to the start of the session.  **Please note: Parent/Guardian must remain on pool grounds during lessons.**
