2020 Utah Business Week Application

Thank you for your interest in Utah Business Week (UBW)! To be considered for the program, please complete the application below. Ask your parent/guardian to review the application information, authorize, and insert their name giving you permission to attend the program. Once submitted, please allow up to a few weeks for more information. The tuition fee for UBW is $100 and can be paid either online or with a check after acceptance into the program. Please note the following eligibility requirements:

ELIGIBILITY REQUIREMENTS:
- Student must not have previously attended UBW.
- Student must be a current high school sophomore or junior (incoming junior or senior).
- Student must have permission from a parent/guardian.
- Student must have the endorsement of a high school teacher, counselor, or administrator.
- Student must have no history of disciplinary problems.
- Student must complete three to four sentences about why you'd like to attend UBW.
- Student must sign the code of conduct.
- Please submit application by June 30, 2020.
