PLEASE READ: Use this form to provide feedback, submit questions and/or report incidents of concern directly to the Langley Minor Lacrosse Feedback & Discipline Committee. We recognize some types of feedback may be difficult to provide, however, it is critical for the health, safety and positive experience of our youth athletes and membership in general that feedback and concerns be heard.

Alternately you can directly contact our Feedback Committee at feedback@langleythunder.ca. The Feedback & Discipline Committee currently consists of two Executive Members, Kara Eastgate (Vice-President) and Michelle McGregor (Director, Administration).

* You must immediately report issue of risk of harm to a person and/or that are potentially criminal in nature directly to the local police by calling 9-1-1 or the local non-emergency number. As a secondary step submit an incident report to the LMLA Board *

Code of Conduct & 24 Cool Down Hour Policy *

Code of Conduct applies to feedback and incident reports and as such shall be accurate, truthful, constructive and provided in a respectful manner no matter the situation. If you are "heated" over the incident you have cooled off for 24 hours before submitting.
