Oak Hall Middle School: Application to the Advanced Math & Science Program (AMSP)
The Oak Hall Middle School Advanced Math & Science Program seeks applications from students who are highly motivated, mature, and have an intrinsic interest in math and science. Applications for the 2019-2020 AMSP are due by Monday, March 25, 2019. Administration will be available to answer any questions concerning the AMSP on Monday, March 18 at 8:00 am or 5:00 pm in the Middle/Upper School Media Center. Section II: Academic Information

Section III: Parent/Guardian Recommendation
In what ways does your child demonstrate high motivation, maturity, and an intrinsic interest in math and science? Why do you feel they are a good fit for our advanced program?
Section IV: Teacher Recommendation
A teacher recommendation form will be completed by the 5th grade teacher of all currently enrolled Oak Hall students who apply. If you are not a current 5th grade student at Oak Hall, please provide a recommendation letter from your current teacher in a sealed, signed envelope. Section V: Written Essay
For currently enrolled Oak Hall applicants, a written essay prompt will be administered in class by the 5th grade teachers. For applicants who are not currently enrolled, a personal interview and assessment will be arranged prior to admission into our advanced program.
