WAIVER FORM: I acknowledge and accept that my son's or daughter's or ward's (as named above) participation in the JUMP League Esports Activity (“Activity”) offered is entirely voluntary and all risks attendant to the activity (including losses that may result directly or indirectly from my son's/daughters/ward's own actions, inactions, or negligence, or from the actions, inactions, or negligence of others, the conditions of the facilities, equipment, or areas where the Activity is being conducted) are freely and voluntarily assumed by my son/daughter/ward and me. I have ensured that my son/daughter/ward understand that it is important for his/her/their safety, and for the safety of the group, that any rules and instructions given by the Activity's organisers, officers, employees, volunteers, teachers, agents, or chaperones of the Activity are to be obeyed at all times. I certify that my son/daughter/ward is in good health and has no physical condition that would prevent participation in the Activity. I consent to emergency medical treatment being given to my son/daughter/ward in the event such care is required. In consideration of your agreeing allow my son/daughter/ward to participate in the Activity, I hereby agree not to hold the Activity's organiser, hosting location, and/or their officers, employees, volunteers, teachers, agents, chaperons, and the Board of Directors liable for:- (a) any expense or loss in respect of personal injury to or death of my son/daughter/ward unless the same is directly and solely attributed to the wilful default of any officer, employee, volunteer, teacher, agent, or chaperone of the Activity's organiser; and (b) any other loss or damage to my son/daughter/ward howsoever occasioned. I agree to indemnify and hold harmless the Activity's organisers, its offers, employees, volunteers, teachers, agents, chaperones, and the Board of Directors and its individual members thereof, from and against any and all expenses, losses, claims, or damages of any kind whatsoever (including legal fees) incurred or resulting from my son/daughter’s/ward’s participation in the Activity, including transportation.I have read and fully understood the terms of this waiver form and its legal significance and hereby consent to allow my son/daughter/ward to participate in the Activity in acknowledgement of the foregoing.I agree that in relation to any legal action or proceedings arising out of or in connection with the Activity, the governing law shall be the laws of the Republic of Singapore, and that the Courts of the Republic of Singapore shall have exclusive jurisdiction to hear, try, and dispose of the matter.
