PSMLA School Group Membership Application
School Group Memberships and annual renewals must be completed via this online form. A minimum of three teachers from one school must join to take advantage of this reduced membership rate.
You will need to know the names, home addresses, phone numbers, emails, and languages of those you are enrolling.
After completing this form, details will be displayed for online payment via credit / debit card or for check payment. For those who need an invoice to submit to an administrative office or if the payment will be made by another party, a copy of this form will be emailed to the address entered below which can be printed or forwarded to the payer.
