Thank you for encouraging your child(ren) to serve the community through our new youth club. This form will collect contact information, age group details, and permissions for participation.

🌙 Parental consent and emergency information are required for participation.

🌙 Membership for each child is $35 which covers the cost of all the events for the year. Please Venmo @gen313 to confirm your child’s registration.
