reBuildit Agency Computer Application Form

Thank you for your interest in our program. If you are an agency or case worker aiming to support individuals facing poverty, Indigenous communities, newcomers, or students, and you wish to request computers on their behalf, please utilize this form to assess their eligibility. Please read below as the process has changed. We recommend grouping your requests by completing multiple forms consecutively to facilitate bulk pickups by your organization.

Once we receive the form, the STEAM Team will review it and contact you to discuss the request further. If approved, we will coordinate a pickup time for the computer from our location in St. Thomas, Ontario. 

Due to overwhelming demand and funding limitations at this time we are only able to offer a  limited number of fully subsidized laptops. If you are an agency with digital support funding from St Thomas- Elgin-London-Middlesex-Oxford you may apply for the $140 subsidized laptop program which are prioritized.

To manage the high volume of requests, we enforce a limit of ONE computer per household.  Please note that there are NO warranties or ongoing IT support provided post-distribution.
