59th Annual Frontier Days Parade Application
This application is to participate in the June 3, 2021 Frontier Days parade. Please note that COVID protocols in place at the time will be strictly observed. This is not only to follow state and county requirements, but to help keep our community safe and well. If you are uncomfortable following required protocols, which may include wearing a mask over your mouth and nose and staying distanced from other parade participants and parade viewers, including not handing out items if not permitted, please do not apply. Thank you for your understanding!

THIS APPLICATION MUST BE RECEIVED NO LATER THAN MAY 7, 2021. NO EXCEPTIONS.

NO sirens or horns are to be used on the parade route, Any units involving animals/horses MUST clean up their staging area before entering parade. YOU must also clean up behind your animal/horse along the parade route. If float exceeds size listed on this application, we reserve the right to replace your entry in a lineup area with more space, thus forfeiting your original parade number. *
Please read & type your name: We hereby make application to participate in the 57th Annual Frontier Day Parade, to take place on May 30, 2019 in Milford, Ohio. We understand that providing automobile accident, personal injury and personal liability insurance is our responsibility, and in lieu of providing an Insurance Coverage Certificate, we hereby certify that in the case of an accident, or any other personal injury we will not hold the Milford Miami Township Chamber of Commerce or any of their personnel responsible for bodily injury, property damage and hereby release any member, officer, trustee, employee, or contractor of the Milford Miami Township Chamber of Commerce for any claim or claims that might arise as a result of bodily injury, property damage or other claims arising out of or as a result of applicants participation in the parade. We further agree to abide by the rules of the parade, including any COVID requirements agreed to between the Clermont County Department of Public Health and the Milford Miami Township Chamber of Commerce. In support of the Milford Miami Township Chamber of Commerce, we will require all participants of our organization to use good safety precautions during our participation. In case of an accident or damages incurred, the Milford Miami Township Chamber of Commerce will in no way be held responsible. *

Please pay via PayPal (info@milfordmiamitownship.com), Venmo (@Milford-Miami), call 513-831-2411 with a credit card number, or send a check to Milford Miami Township Chamber of Commerce, 745 Center St #302, Milford, OH 45150. Your application is not complete until payment is received. Payment must be received no later than May 10 to participate.
