Complete the following form to submit your samples for genetic testing at the NCSU Veterinary Cardiac Genetics Lab. In this form, you will select your test, fill out animal and submitter details, and provide payment information (instructions on how to pay will be provided to you in the payment section of this form). Upon submission, you will receive an email from us confirming that we have received your information. This email will also contain information on how and where to mail your samples to us.
Please have your samples ready to submit BEFORE completing this form. For our genetic testing, you have the option to either submit an EDTA blood sample, or to submit two cheek swab samples for each animal. To submit a blood sample, have your vet pull at least 1mL of blood into an EDTA tube. To submit cheek swabs, please request a swab kit (link:
https://cvm.ncsu.edu/nc-state-vet-hospital/small-animal/genetics/cheek-swab-request/), and we will mail out sterile swab brushes and sample collection instructions via USPS. Samples do not need to be shipped overnight or on ice - standard packaging and shipping is fine.
MAKE SURE YOUR SAMPLES ARE LABELED. All samples must be labeled with the ANIMAL NAME, OWNER FIRST AND LAST NAME, and the TITLE OF THE REQUESTED TEST. Include all of this information on the outside of the swab packaging or blood sample tube.
