Student 1-to-1 Chromebook Repair Form

This form is designed to collect the required information to repair 1-to-1 chromebooks provided by Elk Lake to JSHS students.

Acknowledgement of Repair Costs:

In the Chromebook Policy given to all students upon acceptance of a Chromebook and signed by their Parent (also found here, and on our website Under Forms/Documents) Section 5.2 - Repairs & Fees outlines the costs associated with repairs for specific damaged components.

Estimated costs/fees are listed below and subject to change.
- Charger/Adapter $45
- LCD/Screen $75
- Keyboard/Touchpad $70
- Plastic body parts (top/bottom cover, bezel etc.) $40 each
- Damages not to exceed $325 or the current cost of a new Chromebook.

By filling out this form you are agreeing to reimburse Elk Lake School District for the cost of the damaged components listed above. Upon reciept of the required repair fees, Elk Lake School District will replace the damaged chromebook.
