Please complete this form to apply for a place at a school run club for children in Years 1 to 6.

You will need to complete a form for each child.

These clubs will take place on the school field (or Upper KS2 Playground)

Due to the popularity of these Clubs, please do not send your child unless you receive a confirmation email that your child has secured a place.  In the event of oversubscription, children will be selected on a random basis, with priority given to those who have not attended previously attended.
