Parents/Guardians - Please complete and submit this form at least 15 school days prior to the application deadline. If we do not receive this completed form, we will not be able to submit your student's educational information/official EHS transcript that was requested through Naviance.

Please note - Once you have completed this form it may take up to 48 hours to reflect in Naviance. Accounts are not updated on non-school days. You only have to complete this form once.

The transcript of a student's Essex High School record includes the student's courses, grades, credits, grade point average (GPA), and personal identification information. Official transcripts are sent directly from Essex High School to the receiving institution or program.

Under FERPA (Family Educational Rights & Privacy Act) we require your permission to submit educational records to outside parties (colleges, scholarships, employers, NCAA, etc.) for students under the age of 18.

By completing and submitting this release form, you are allowing the Essex High School Counseling Office to submit your student's educational records/transcripts to the organizations they have requested through Naviance to receive them.
