Working in teaching, research labs or Chemistry Service Centers with chemicals is prohibited until an active Chemical Safety Training Certification is obtained. The Safety Training is a University requirement and must be completed with a passing score of 80% (or 80 points). The certification is only valid for one year and if you are continuing research after this time, you will need to complete a refresher quiz.

NOTE: You must have an active SJSU ID and/or SJSU employment appointment before submitting this form, unless you are a non-SJSU Affiliated Volunteer.

1. Complete this form to request access to view the Chemistry Safety Training review material on Canvas or an email of study material  for non-SJSU affiliated Volunteer.  An SJSU or Google email address is needed.
2. The invitation will be sent within 3 days through Canvas for you to accept access. If after 3 days you have not received it, contact Kitty at ngoc-huong.nguyen@sjsu.edu.
3. Review the instruction manual on Canvas.  Note that the quiz is online only.

5. Faculty/Staff/TA/Grad Student/Student Assistant working with chemicals: Please email Kitty to request the quiz access information.  Include your title {Faculty/Staff/TA/Grad Student/Student Assistant}.
6. Volunteers will need to email Kitty so that she can send the quiz via the Google classroom link.

IMPORTANT: You will have 10 days from the date you sign up for review materials to complete and pass the quiz.
