Student Commitment Signatures *
Type the name below to sign. We, student and parent(s), have read the information contained in this form for the Worthington Kilbourne High School Marching Band. By signing this contract, we are making a commitment to join this organization. We understand that by joining this organization, the student agrees to abide by all the rules of our band handbook (found on our website in our registration documents) and school. We understand that being a member of such an elite ensemble is a privilege and not a right. Furthermore, the student plans to attend all scheduled rehearsal and performance throughout the summer and fall as listed on the official calendar of the marching band. We understand that should a student decide to leave the WKMB, the Cancellation Policy found in our marching band handbook shall be adhered to strictly without exception. We understand that the student agrees to follow the established Co-Curricular Drug and Alcohol Policy and maintain academic eligibility requirements set forth by the Worthington City Schools administration. We understand that the student must provide a completed physical card to be eligible to participate. We also understand that we will be responsible for payment of any lost or damaged uniforms/parts for the uniform assigned to the student. We understand that failure to observe these terms may result in the student not being able to participate in some or all of our Marching Band events, performances or rehearsals.
