Treasured Viking Funds Request
This form is for Lakeview families needing assistance with school items and activities such as spirit shirts, Field Day shirts, 5th Grade class shirts, school supplies, a yearbook, club dues (like Choir) and more. Your form will be submitted to the Grants & Scholarships Coordinator for review. Your information will remain confidential and only people who are required to know will be allowed access to your information.

Please note your request will need to be submitted at least 2 weeks prior to the deadline (of the event / item) in order to process the payment information and order properly. Keep in mind the form will be updated throughout the year as more opportunities arise (e.g. Field Day shirt). If you are needing assistance with something else school-related, please submit your request and our Grants & Scholarships Coordinator will assess for eligibility on a case-by-case scenario.

In order to provide for more families, there will be a limit of $40 per student. If you have requests for multiple children, please fill out a separate form for each child.
