This form is ONLY for those paying for the Conference by School Purchase Order. This form will generate an email to you that contains all of the information entered into this form. Within a week of completing the form, you will be emailed an invoice to present to your school so that the purchase order can be completed. It is your responsibility to ensure the invoice is paid.
