Local Scholarship Recommendation Letter Request Form

Use this Google form to request your recommendation letter(s) be sent directly to any of the 2024 local scholarships listed on our MHS scholarship website.

Submit a separate form for each scholarship that requires it. We will use the recommendation letter(s) that your teacher and/or school counselor wrote for your college applications. If you need more than one letter sent to the same scholarship, please indicate it on the form.

If a scholarship requires a transcript, you may send them your unofficial transcript copy along with your application. You can obtain it in Aspen in the Pages Section under Published Reports.
