The student verification form sent to you identifies the information HDSB has on file in its student information system. Prior to starting please ensure you have reviewed the information and identified the information that will need to be updated by completing this form. There is no ability to save progress and complete at a later date.
Personal information on this form is collected under the authority of the Education Act, R.S.O. 1990, c. E.2
and will be used only for purposes related to the regular operational and administrative requirements of the HDSB. Questions with
