Celebration Education Classes Registration

PLEASE READ THESE INSTRUCTIONS!

1. Complete one form for each child that will take our classes. See the classes by location at CelebrationEducation.com .

2. If you haven't already done so, get a membership at LearningOutsideTheBox.com so we can add you to your child's class group on that site. Any membership is fine, but the family membership will include weekly at-home "Fireworks" to go with the thematic classes.

3. Whether you pay out of pocket or use charter school funds, we will send you an invoice for the amount due for the class(es) your child will take.

Names of Classes - List each class you would like your child to take. *

If using charter school funds to pay for classes, please name the school. Purchase orders or certificates must be received before the first day of class.
