This form is a means by which safety concerns can be reported to ChemGSO. Reports are anonymous, but you have the opportunity to include your email address if you choose. Including as much information as you can will help us solve your concern. If you choose to include your contact details, we will make sure to request your permission before sharing your name in connection with your concerns when addressing the relevant parties. These concerns include relevant safety issues within the department among research groups, whole divisions, departments outside of chemistry housed in Gray-Little Hall, or teaching labs within our department.
