V súlade s platnou legislatívou zaslaním tejto prihlášky udeľujete Dopravnému úradu a LPS SR, š. p. súhlas so  spracovaním svojich osobných údajov pre účely Semináru o bezpečnosti v civilnom letectve v rozsahu meno, priezvisko a e-mailová adresa. Poskytnuté osobné údaje účastníkov budú po skončení semináru vymazané. Súčasne prihlásením sa na seminár súhlasíte, že Vaše osobné údaje budú zhromažďované spoločnosťou Google prostredníctvom Google forms na nevyhnutne potrebný čas semináru.  V týchto zásadách je vysvetlené, ako spoločnosť Google zaobchádza s osobnými údajmi. Zároveň beriete na vedomie, že máte právo na opravu nesprávnych, neúplných a neaktuálnych osobných údajov a ďalšie uvedené v § 28 a nasl. platného zákona o ochrane osobných údajov.

Application form for Civil Aviation Safety Seminar
In accordance with relevant legislation, by sending this application form you grant the Transport Authority and LPS SR, š. p. consent to the processing of your personal data for the purposes of the Civil Aviation Safety Seminar to the extent of name, surname and e-mail address. The participants personal data will be deleted after the seminar.  At the same time, by signing up for the seminar, you agree that your personal information will be collected by Google through Google forms for the necessary time of the seminar.
