With this form you are able to request items to help to reduce risk and increase safety in our communities. We have the following kits available:

Narcan Kit - To reverse an opioid overdose with medication in a nasal spray; kit comes with gloves and a face barrier
Drug Test Strips - ( kit includes Fentanyl and Xylazine testing strips and other supplies needed for testing)
Lock Bag- (black cloth pouch 8" x 7.25" that has a combo lock for safer medication storage)
Lock Box - (gray cloth box 9.5"H X 4.5"H X 4"D that has a combo lock for safer medication storage)
Medication Disposal Kits - (Environmentally friendly way to dispose of unused medication at home)
Safer Sex Kits - (kit includes male and female condoms along with lubricant)
