Parental Agreement For Transportation to Alternate Existing Locations
This form must be completed every school year the service is requested.

CPSB Transportation, at its discretion, may provide transportation for eligible students to and from approved alternate locations such as daycares and learning centers when space is available on the requested bus. An eligible student is one who meets the criteria for qualifying for school bus transportation as outlined in Louisiana School Transportation Bulletin 119 Chapters 19 and 21 and in Caddo Public School Board Policy. An alternate location must be within the student's school district and over a mile from the school site to be considered for approval unless agreed upon in an IEP.

CPSB Transportation reserves the right to suspend any or all services to alternate locations at any time. The parent or guardian listed on this agreement will be given a notification period of no less than five (5) days of a suspension occurring during the current school year. Services agreed upon in an IEP may be reviewed each year and terminated upon re-evaluation of the IEP. Notifications may come in writing or by verbal contact from the school site, the school bus driver, or from the Transportation Department. If for any reason there is not a valid contact number available, the date and time of notification will stand as the moment of attempted contact. Notifications are not required during summer revisions of school bus routes.
